A 45 yr old male who is a driver by 
occupation ,resident of thummalaguda came to the OPD with itching sensation and shortness of breath.
Cheif complaints
Shortness of breath
Cough with expectoration
Itching sensation all over the body 
Increased urine output (15 times/day)
Increased frequency of passing stools.
History of present illness
Shortness of breath since 30 days
Itching all over the body  since 30 days
Cough with  expectoration since 1 week
Increased urination since 1 week
Increased frequency of passing stools since 1 week
History of past illness
No h/o DM,HTN,
No h/o previous hospitalization
No h/o any surgeries.
No h/o previous medication.

Camp case 80 yr old male  CHEIF COMPLAINTS: Came with c/o giddiness since 15 days. HOPI: Pt was apparently asymptomatic 15 days back and then had giddiness which was  episodic daily once ,mrng before lunch a/w blurring of vision during that episode and relieves after taking food.
