altered sensorium due to electrolyte imbalance

A 60 yr old female resident of Nalgonda came to casuality with complaints of decreased urine output and loss of speech.

COMPLAINTS AND DURATION

k/c/o DM since 20 yrs
c/o  Generalized weakness and bodypains since 2 months.
c/o pain abdomen since 8days
       Drowsiness since 3days 
       Decreased urine output since 2days


HISTORY OF PRESENT ILLNESS

pedal odema
abdominal distension -(+)
Shortness of breath
H/o usage of aceclofenac since 3 days

HISTORY OF PAST ILLNESS

NO HTN
NO Thyroid Abnormalities
NO Epilepsy
NO CAD
NO CVA


PROVISIONAL DIAGNOSIS:
ALTERED SENSORIUM 2° to HYPONATREMIA.
C  AKI
C  HEPATITIS
C RENAL CALCULI
C DM II with uncontrollable sugars.
